Where is death registration/death notification form reporting? Also, what do you bring when submitting a report?

Notification and certification (family register, resident certificate, etc.)

The following are what you bring and the notification destinations.

For more information, please contact the ward office Family Registry Division Family Registry Section.

What you bring

・1 death registration/death notification form (Notification form is common throughout the country.)
・One death certificate or one postmortem certificate (prepared by a doctor). The right half of death registration/death notification form is death certificate, etc., so I will ask you to prove it there.)
・Please submit fire burial permission application together to perform cremation. (If cremation has been completed, death registration/death notification form has been notified.)

notification period

・Within 7 days from the day when the fact of death is known
・In the case of death outside Japan, within 3 months

Notification destination

The place of death and the location of the applicant (relatives or cohabitants) or the ward where permanent domicile is located (either one is acceptable) will be the ward office Family Registry Division Family Registry Section.
(It cannot be handled at the administrative service corner or city hall.)
