Where can I get a certificate of acceptance for the family register notification?

You can get it at the window of the municipal office where you notified family register.
When we report in Yokohama-shi, we can take in ward office which reported. (It cannot be taken at the city hall.)
You can also request it by mail.

・350 yen per copy
(Only for notification of marriage, divorce, adoption, adoption separation or acknowledgement, 1,400 yen per copy is created using high-quality paper (B4).

・It is limited to applicant who submitted the notification of family register. If proxy/agent is requested, a power of attorney must be submitted.

・You will need to present your photo identification documents (driver's license, passport, my number card, etc.) to confirm your identity in applicant.
・If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identity verification document or ask questions.
You must also submit a power of attorney if proxy/agent is requested.
