When requesting a copy of the family register's tag by mail, enclose a request form, a fixed amount of money exchange / ordinary exchange or cash for the fee, a reply envelope with a stamp, and a copy of your identity verification document, Please send it to the office.

Items to be sent

  • Request form (requester's Address, name, Phone number, permanent domicile, name of the first person, name of the person who proves)

※You can also download it from the city website.
※In principle, copies of family register's tag are omitted from permanent domicile and the name of the first person. If you need to display it, please indicate it on the invoice.
※In the case of a request from a corporation, please indicate the name of the corporation, the name of the representative, and the location, and seal the company seal.
※In the case of a third party request, the use of the invoice (reason for needing a certificate) must be stated. In addition, please enclose "Documents that can confirm the purpose of use". In addition, the certificate will be issued only to those who need it.

  • Fixed amount of small exchange / ordinary exchange (issued at post office) or cash (registered cash) for the fee (300 yen per copy)
  • A reply envelope with a stamp attached and the destination address filled out
  • A document that can confirm the right of proxy, such as a power of attorney, if requested by the person. However, direct relatives are not required.
  • A copy of identification documents (My Number Card, Driver's License, employee health insurance Certificate of Eligibility or Health Insurance Card, etc.) containing the claimant's Address

※When sending a copy of employee health insurance's qualification confirmation or health insurance card, please make sure that the insurer number and insured person's symbol and number are not visible in black.
