How do you fill in divorce registration form?

• My husband and wife must sign it.
• In the case of divorce in court, the petitioner will be applicant, so only one husband or wife will be applicant.

(Please notify the petitioner within 10 days from the date on which the divorce is established or finalized. If it is 10 days after the establishment and confirmation, you can also submit a notification from the other party. )

• Please sign by two adults other than applicant.
• In the case of divorce in court, it is not necessary.

• If the couple has a minor child, be sure to decide which husband or wife will be the custody.

(It cannot be a married couple's parental authority/power. Also, divorce registration form alone does not transfer her child family register. It is necessary to submit a separate report.

• For those who are not married in head of family register, please select and describe how to do family register after divorce. (Please choose to return to family register before marriage or create a new family register.)
• If you use your current Mr. after divorce, you will need to submit a separate notification from divorce registration form, so please do not fill in this column.

(If you do not fill in this column, you will need to submit a notification under Article 77-2 of the Family Registry Act to the name of a married person at the same time as divorce registration form. Please describe the name of your parents and your relationship with your parents. (Please indicate the name of the adoptive parent and the relationship with the adoptive parent in the other columns.)

• Please describe Address, which is currently registered as a resident.
• If you are going to change Address, etc. at the same time as divorce registration form and you want to submit a notification to the new Address area, please fill out the notification form with the new Address and submit a notification regarding the change of Address.
