What do you need when you or your family go to get a copy of the family register tag?

We can request at window of ward office, administrative service corner in Yokohama-shi or convenience store.

Please bring identification documents with a photo attached, such as my number card, driver's license, passport, Basic Resident Register card with photo, etc. of the person coming to the counter.
※If your identity verification document is an identity verification document that does not have a photo attached, such as a employee health insurance qualification confirmation or a health insurance card, or a certificate issued by an organization other than a public office, you may be asked to present other identity verification documents or ask questions.
※If a family member is requested by a person who is recorded on the family register's tag, his / her spouse, or a direct relative who does not fall under any of the direct relatives, please describe their use in detail. If the use is not clear, we may ask you to provide explanatory materials.
※In addition, a certificate cannot be issued if it is not recognized that there is a justifiable reason. Also, a power of attorney is required if proxy/agent is requested.
There is a fee of 300 yen per copy.
(If you get it at a convenience store, it will be 250 yen.)

If you are in Yokohama, both Address and permanent domicile, you can use your My Number Card to obtain a certificate that shows you through a multi-copy machine installed at convenience stores nationwide.
To use it, you will need the PIN (4 digits) of the digital certificate for user certification.
The usage time is from 9:00 to 17:00 on weekdays (excluding system suspension days).
There is a fee of 250 yen per copy.

If you have a My Number Card, a smartphone compatible with My Number Card, or a credit card, you can apply for a certificate that describes yourself online.
To use it, you need the PIN of the digital certificate for signature (6 to 16 digits alphanumeric characters).
In addition, 300 yen per copy + postage will be charged.
