Where does divorce registration form (divorce by judgment of district court for mediation, etc.) report? Also, what do you bring when submitting a report?
When submitting a divorce by trial (divorce by mediation, trial, judgment), please bring and report are as follows.
Please refer to the separate item for how to write divorce registration form.

What should be brought to you during the divorce registration form trial
- 1 divorce registration form (Notification form is common throughout the country.)
You can hand out divorce registration form paper and instructions on how to write them at the ward office Family Registry Division Family Registry Section or administrative service corner. (We do not hand it over at the city hall.)
In addition, we can download from page of electronic application, report service of Yokohama-shi website.
- A copy of the mediation report, determination of family court, and a copy of the judgment document and one copy of the confirmation document (You may be a messenger other than applicant.)
- If the person who is not the first person of family register is still married after divorce, a separate notification is required. (Please notify us within 3 months from the date of divorce establishment.)
- If the couple has unmarried children, a separate notification is required when transferring the child to a new family register after divorce. (In divorce registration form alone, the child family register will not be transferred.)

- Within 10 days from the date on which divorce is established or finalized (only notification from the petitioner)
- If it is 10 days after the establishment and confirmation, you can also submit a notification from the other party.
Notification destination
- It will be the "Ward Office Family Registry Division Family Registry Section" in the location of the person in applicant or in the ward where permanent domicile is located. (It cannot be handled at the administrative service corner or city hall.)
- Even if you don't have Address or permanent domicile, you can notify the ward office where your home address is located, such as when you are staying at your home. (For details, please refer to the "Ward Office Family Registry Division Family Registry Section" where you bring your notification.)
- In the case of delivery other than the ward office Hours, documents will be kept at the ward office's night/holiday reception desk, and the contents will be checked in the next Hours belt, and then formally processed.
If it is not officially accepted, we will contact you, so be sure to include the Phone number that can be contacted during the daytime on weekdays in the form.)
