How do I return my number card?

If you find a lost card after reissuing your My Number Card due to loss, etc., please bring your My Number Card and notify the ward office Family Registry Division Registration Section of your ward office.
In addition, we also accept voluntary return of My Number Cards. Please note that a fee (1000 yen) will be charged to obtain a My Number Card again after notification by voluntary return.

2nd fourth Saturday: From 9:00 to 12:00 (noon)

Notification of persons under 15 years of age or adult ward
If you are under 15 years of age or an adult ward, please prepare the following documents and notify the legal representative (parental guardian, adult ward, etc.)
- My Number Card
- proxy/agent's identity verification documents
- Documents proving the right of representation
- Under 15 years old: Proof of all matters in family register
- Adult ward: Adult Guardian Registration Certificate
