What should I do if I have lost the delivery notice (postcard) of my number card?

If you know the reservation ID listed on the upper left of the back of the delivery notice (postcard) (if you can make a reservation), you can receive your My Number Card if you can bring one of the following mochi .

If you do not know your reservation ID, you will not be able to accept reservations, so please contact your ward office Family Registry Division Registration Section. You don't need to make a reservation only if Nishi Ward residents receive it in Nishi Ward Office.

• My Number Notification Card or Personal Number Notification and two types of identity verification documents (without a photo is acceptable)

※Please fill in the documents called the inquiry and response form at the counter.

• Two types of identity verification documents, including a face photo issued by a public office such as a driver's license.

If you do not have any of the above, you will need to reissue a postcard, so please contact your ward office Family Registry Division Registration Section.
