Where is birth registration form reporting? Also, what do you bring when submitting a report?

The following are what you bring and the notification destinations.
What you bring
1 birth registration form and birth certificate
・The notification form is common throughout the country.
・applicant is a father or mother if born during the marriage of parents, and a mother otherwise. If your parents are unable to become applicant, please check with the ward office Family Registry Division Family Registry Section.
・You may be a messenger other than applicant.
・The right half of birth registration form is birth certificate, and you will have to be certified by your doctor or midwife. If you are unable to prove your doctor or midwife, check with your ward office Family Registry Division Family Registry Section.
2 Maternal and Child Health Handbook
Characters that can be used for the child's name
There are restrictions on the characters that can be used. For more information, please check with the ward office Family Registry Division Family Registry Section.
(The characters that can be used are kanji for common use, kanji for personal names, hiragana, katakana, etc.)
notification period
・Within 14 days from the day of child birth
・In the case of child birth outside Japan, within 3 months
・If you are abroad and have acquired a foreign national, you must submit a nationality reservation at the same time as birth registration form within three months from the date of child birth.

Notification destination
・The location of child birth and the location of applicant (father or mother) or the ward where permanent domicile is located (either one is acceptable) will be called Family Registry Division Family Registry Section.
(It cannot be handled at the administrative service corner or city hall.)
・Even if you don't have Address or permanent domicile, you can notify the ward office where your home address is located, such as when you are staying at your home.

When it is delivered outside the ward office Hours
・We will keep the documents at night and on holidays at the ward office, check the contents in the next Hours, and then formally process the reception. If it is not officially accepted, we will contact you so be sure to include the Phone number that can be contacted during the daytime on weekdays.
・In addition, there is a page in the Maternal and Child Health Handbook to prove that you have done birth registration form, but if you send it outside the ward office Hours, you will not be able to prove it on the spot. Please confirm in advance the handling of the certificate of the Maternal and Child Health Handbook with the ward office Family Registry Division Family Registry Section.
