Notification and certification (family register, resident certificate, etc.)

You can get it at the window of the municipal office where your permanent address is located. If there is a permanent domicile in Yokohama, you can get it at all the ward office Family Registry Division counters in the city (administrative service corner and city hall are not available.) 。 You can also request it by mail or online.
Please refer to the City website for inquiries.

If you have a permanent domicile in Yokohama, you can handle it at all the ward office Family Registry Division counters.
(We do not handle at the administrative service corner and city hall.)

300 yen per copy

Items to be brought at the time of window request

You will need to present your photo identification documents (driver's license, passport, my number card, etc.) to confirm your identity in applicant.
If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identity verification document or ask questions.
Also, if proxy/agent is requested (including relatives listed in the same family register), a proxy must be submitted.

Reception hours (Family Registry Division Family Registry Section, each ward office)

Monday to Friday: 8:45 am to 5:00 pm
2nd and 4th Saturday of every month: 9:00 a.m. to 0:00 p.m.
(In Reception hours, lunch breaks are also accepted, Saturday (except 2nd and 4th)
Excludes Sunday, public holidays, public holidays, national holidays, and New Year's holidays (December 29 to January 3 of the following year)

※Please use the form given by the marriage information service / marriage consultation company. If it is not available, you can also download it from the city website.

  • Copy of identity verification materials (My Number Card, Driver's License, Health Insurance Card, etc.) containing Address

※When sending a copy of your My Number Card, please send only the surface.
※When sending a copy of your health insurance card, please send the insurer number and the insured person symbol / number by black paint etc. so that they cannot be seen.

  • Fixed amount of small exchange, ordinary exchange (issued at post office) or cash (registered cash) for the fee (300 yen per copy)
  • Reply envelope (fill in the reply address and attach a stamp)

When billing online

If you have a My Number Card, a smartphone compatible with My Number Card, or a credit card, you can apply for a certificate that describes yourself online.
To use it, you need the PIN of the digital certificate for signature (6 to 16 digits alphanumeric characters).
In addition, 300 yen per copy + postage is required.
